I need help for my sunken cheeks

Dear Dr Thorne

I am 38 years. I have sunken cheeks and a weak chin.

I have had non permanent and semi permanent fillers like Sculptra. The non permanent fillers worked well for a few months then went. The Sculptra was too imprecise.

I am looking for a filler that will last many years so I can move on and not have frequent injections.

Can you help? I have heard that Aquamid and Silicone can provide a lasting solution. Do you use these? What would be your recommendation.

Hello there I do not use permanent fillers such as Aquamid as there are many risks of complications. If you are happy with the non-permanant fillings, but feel they last only a few months then Radiesse is a longer lasting option. It is a calcium hydroxyapetite filler which lasts around 18 months. I would not recommend using a permanent filler because your face changes with the ageing process around the filler and the results after several years can be aesthetically displeasing. Prices start from £650 for 1.5ml of Radiesse.

Would it be possible to reshape my nose with dermal fillers?

Hiya, I’ve been looking into possibly undergoing a non surgical nose reshape, I’ve got a slight bump on the bridge of my nose that I’d just like flattened, to create a more flat nose shape from a profile view. I’ve been researching different non surgical treatments, including Radiesse and Aquamid, I wanted something that was as long lasting as possible so I can avoid having to have the procedure re-done. I’m a 19 year old male, I was wondering what options I have, obviously trying to keep the costs to a minimum but still keeping a great and long lasting result. I look forward to hearing from you.

To be honest right from the start; I have no personal experience of nasal reshaping with fillers and it is not something that I would do. I would suggest however, that before you allow someone to inject a long-lasing filler such as Aquamid into your nose it might be a good idea to try a HA filler first. Although this is not a long term solution it would allow you to see whether you like the effect. If it wasn’t quite what you were hoping for then at least it would slowly disappear or you could have hyalase injected to disperse it.
You are obviously doing some research, which is good. Be very careful and good luck.

Is it possible to refill lips which have been treated with Aquamid?

Hi Dr,
Can one who has had lip augmentation with Aquamid 2 years ago , now refill her lip with a shorter lasting gel such as a hialloronic acid filler over it ? It seems that the previous gel has been maximally disappeared unless a few palpable micronodules in lip contour . and in general what could we do with the remaining gel nodules in treated areas ? what is your opinion about ALAYNA fillers ? Thank you so much

Hello there,
I am very conservative when it comes to fillers and particularly lip fillers where I would only use a hyaluronic acid (HA) filler. Although not long-lasting they are very safe and rarely cause problems. In addition they can be dispersed with hyalase.
The question about injecting HA over Aquamid is a difficult one. All I can say is that if the Aquamid has been very settled for a number of years then it might be OK. There is always a risk of the HA triggering problems with the underlying permanent filler.
If the Aquamid nodules are not causing you any bother then I would leave them alone.

I would like a permanent filler, I’ve heard Aquamid is the best but do you know of any that are equally as good?

Hi there, I am 61 and I would like a permanent filler to fill in deep lines above my upper lip & creased going downwards from edges of my mouth. I am not interested in short term fixes. I have read that Aquamid is recommended as the best, or do you know of any others equally as good. Also my main concern is finding a very experienced surgeon in this field, as I have read that the permanent fillers need to be injected deeper into the skin that the temporary ones, and preferably in the Liverpool / Manchester area. How do I find one please. Thank you!

I would suggest that the best way to find a properly trained and qualified surgeon would be to look for a member of BAAPS. You would also need to check that your chosen surgeon is also experienced with non-surgical, injectable treatments. For obvious reasons many excellent surgeons tend to concentrate on surgical solutions. Alternatively you could look for a Cosmetic Doctor (rather than Cosmetic Surgeon). Cosmetic Doctors, like myself, are trained and experienced in a range of non-surgcal options including dermal fillers.

Personally I am very hesitant to use permanent or semi-permanent fillers. In the past, most of the long-term problems that have occurred have been with this type of filler. Some permanent fillers have late-onset complications; this means that everything seems to be OK for a few years and then out of the blue serious inflammatory reactions can appear. Hyaluronic Acid filers such as Restylane and Juvederm do not have this problem. Restylane has been used millions of times over the last 15 years and has an unparalleled safety record.

The advantage of HA fillers is the low incidence of complications and we used to believe that their disadvantage was their lack of longevity. Recent scientific studies have shown that although the HA does slowly disappear it does stimulate some natural collagen production. This partly explains why in some people HA fillers seem to last a long time and why people returning for top-ups often require smaller volumes than initially used.
