What filler would be best for nasolabial lines?

Hi dr,
I would like some advice on which filler would be best for nasolabial lines. I am 19 years old and don’t have wrinkles but recently the expression lines going down from my nose to mouth have become deeper, and it has given my face a more gauntish, harder look, and I have very faint marionette lines because of this too. Where as before my face looked alot fuller. I think this is from poor diet, smoking and drinking too much coca cola.The expression lines don’t look even either, and the one on the left side of my face goes straight down, whilst on the right side it is curved and slightly softer around the mouth. I am choosing between restylane and juvederm. I like juvederm because I read it gives a more softer look than restylane, which is what I would like, but am worried because I heard juvederm tends to spread out more and go into other areas. I don’t want it to spread to the cheek area as I already have very full cheeks and it will look weird. I want it specifically for the nasolabial folds, too soften them. I want a filler that will look natural and subtle and just make my face look the same as it used to and not really different.

Hi There,

Both Restylane and Juvederm are excellent fillers and wold do a great job. I don’t think that Juvederm will spread any more than Restylane.
It’s probably more important to choose your practitioner carefully.

How can I treat my aged and sunken face?

Hi there,
I have got quite sunken cheeks and more lines on my face since loosing a lot of weight. Whilst my body look the best it ever has my face seems to have aged. I have tried a few creams and serums but they haven’t really made any difference to my face at all. I have been looking on the net at dermal fillers and things and I don’t really know very much about them or where to begin really. I just want my face to look younger, not a great change to to plump it out abit. Can you give me any advice about what creams to use and what fillers you think would help with my face?

Hello there,
Although there are good creams to improve the tone and texture of the skin surface they will not replace lost volume or plump the skin. This is where hyaluronic acid (HA) dermal fillers come in. They can not only subtly restore lost volume but can be used to provide a more widespread plumping and re-hydation of ageing skin. My personal preference is to use either Restylane or Emervel but there are other HA fillers. There are a number of ways in which HA fillers can be used to achieve this; you would have to discuss this with a reputable Cosmetic Doctor.

I am worried about getting fillers for my upper lip

Hi Dr, I would like some advice about getting fillers for my lips. I have quite a good sized lower lip, but my top lip is very thin. So, generally i dont draw attention to them by wearing gloss or lipstick. I would like to increase the volume of my top lip, just so it looks better with my lower lip. The problem is that I am worried it will go wrong and I think the celebrities I have seen have it look dreadful. I don’t want to look like a duck, or for them to look unnatural.I live in Guildford don’t know where to go.Thanks Dr Forrester for your time.

Hi There,
The media gives a lot of exposure to over-done celebrity lips and so called ‘trout pouts’. Naturally this leads people to think that it is always done that way. Of course, if someone has subtle and natural lip filler you wouldn’t even notice. You probably see them every day without a second look.
The lips are a sensitive area and so some initial swelling and bruising may be apparent for a day or so.

What treatment would be best for my facial lines?

Hi Dr Forrester,
I wondered if you could give me some advice?I have some lines above my eyebrows and also around my eyes. I was thinking about having some sort of fillers as I am 38 and want to keep on top of any lines or wrinkles. What fillers would you recommend to fill out these lines? Or do you think Botox might be better?I would like a natural look as I work as a teacher and wouldn’t want to scare the children! I look forward to hearing from you.

Hi there,

Lines in those areas are usually ‘dynamic’ lines. That means that they get deeper during facial movement and smooth out when the muscles are relaxed. In this situation Botox is usually the best option. When subtly done Botox will still look natural and your pupils won’t notice.
It may be that fillers could be an option but I could only really give an opinion face-to-face.

What can I do about the bad treatment I received?

I’m an Italian woman,54 years old. I made filler 4 week ago in Saudi Arabia(Jeddah)with a German doctor,that come here . The filler made me infection,the doctor don’t give me nothing for help me,only cream for herpes. So i go to another doctor that give me two antibiotic and my face now its better but not well.Its possible write to the company  for said that its better control who come in the word for don’t do the same with another person?

Hello there, I’m sorry to hear of your problem.
It is very important to check the experience and qualifications of the practitioner administering the filler injection. It is also important to confirm exactly which brand of filler is being and that it has been sourced from an approved supplier. Lastly I would also advise that you choose a practitioner who offers a comprehensive follow-up service and provides you with contact details in case problems occur.

Are there any fillers that would be good for the neck area?

Hello Dr Forrester,
I have a problem with my neck. My face so far doesn’t look bad at 34, but my neck is starting to get quite a few lines . My question is whether you think that dermal fillers would work for my neck? Also is there anything I can do to prevent further lines? I don’t sit in the sun or use sunbeds etc. Are there any creams or serums that I could use? I normally use body lotion and also whatever moisturiser I put on my face. Also, do you perform fillers on the neck area? Thanks for your time and i look forward to reading any advice that you can give me.

Apologies for my delay in replying. I have been at IMCAS, a cosmetic conference.

The neck can be a problem area as the skin is quite sensitive. Although you don’t sit in the sun I would recommend a sunscreen on the neck and face 365 days a year. Even if it is cloudy there are still UV rays out there.

I would recommend a Vitamin C serum on the neck and face. Choose a serum that has at least 10-15% vit C; don’t rely on creams that contain vitamin C as they rarely have enough to be effective. Remember that this is not an overnight miracle but a long term strategy. I think that the new Medik8 serum called CE-Thione is excellent. In addition to a sunscreen and vit C you should consider a retinoid but because the neck is sensitive you need to choose carefully.

Fillers have a limited role in the neck but a course of something light like Restylane Vital can help. Lastly I am a big fan of INTRAcel which is a type of radio-frequency needling that is beneficial over the whole face and neck.

Can permanent fillers be used to treat Polands disease?

Are permanent fillers still available to treat Polands Syndrome? I previously had the Bio Alcamid injections to mimic the missing pectoral muscle which proved satisfactory for six years until it migrated into a lump.
This was subsequently removed.
I have no fat for fat transfer and would prefer a permanent filler if one could be recommended.
The injections were given by an extremely competent professor but unfortunately he had no knowledge of the filler and managed to do his best.Since then he has retired and I am unable to ask his help again.
I have also questioned the idea of a silicone implant but have been told that this will not work as the edges can be seen.
Is there another filler other than the Bio Alcamid that could be used or would this filler work better in the hands of an expert in the field?

Hello there,
Permanent fillers are slowly failing out of favour because of the high incidence of problems. I’m not sure that you should discount silicone implants. Pectoral silicone implants have been used to reconstruct Polands Syndrome. Their success depends on your degree of Poland deformity, the careful choice of implant size and doing some research to find a good surgeon. The advantage of a solid silicone implant is that they are usually much better tolerated and long lasting compared with injectable permanent fillers.

Can the Bio-Alcamid be removed from my pectoral area?

Dear Dr Forrester,
in 2010 I had pectoral implants which were far from perfect, the surgeon based in the UK, then added a total of 60mls bialcomide in an attempt to even out the effects. Two years on I have since had the implants removed and corrective surgery in the US, however the US surgeon was unfamilar with the product and therefore reluctant to remove it. I now often have tenderness where the bialcomide sits and am keen to have it removed. I have three questions. 1, can it been succesfully removed? 2. can I have it done in the uk? Is it a expensive and difficult proceedure? I would welcome your comments

Firstly let me say that I have never used Bio-Alcamid and therefore am neither an expert in its use nor its problems. As you probably know permanent fillers have become much less popular because of the side effects. I understand that removing these fillers is not easy. I do know a doctor who is very experienced in the use of Bio-alcamid. I would not want to give his name out but if you email me directly I will give you his details.

Would Restylane be safe to put in my nose?

Hello dr Peter, first sorry my bad english, to ten  years ago i put artecol in the wrinkle the noise to the mouth.Now I want to put something like restylane or another thing? But Doctor say that is dangerous , i don’t know what i do, please help, what can i put? im so sad to see my face like this way and i can t do something:( please answer my question and i hope you understand my bad english! thanks for your attention

The problem with all semi-permanent fillers is that they can suddenly cause problems years after they have been injected, even if everything had previously ben very settled. Restylane is a very safe hyaluronic acid filler. However, your doctor probably said that they wouldn’t inject it where Artecol had been used just in case it caused it triggered a reaction with the Artecol.

Will dermal fillers wear off and make my face droop?

Hi, I am wondering if you can help. I was having a chat with friends regarding dermal fillers, Botox etc as I am considering having both done for forehead lines and nose to month lines. However they were against it saying that once it wore off the face would just drop and the lines would be worse! However I am thinking having it earlier rather than later would help as it would stop the wrinkles constantly contracting as the face wouldn’t move the same so they’d actually be less noticeable once it wore off compared to a woman the same age who had never had treatment am I right? Thanks

There is no evidence that things will be worse when Botox or fillers wear off. Equally, there is not a lot of evidence that they have a long term benefit, although I suspect that they do. There is some evidence that hyaluronic acid (HA) fillers such as Restylane or Emervel improve the texture and hydration of the overlying skin. Also HA fillers have been shown to stimulate the body’s natural collagen production.

Certainly having them earlier rather than later means that you wold probably need smaller amounts and the results are likely to be more subtle and natural.
